For clients who are in good standing and have been with us for six or more months, the BonaVita Membership Program is available to you. For a minimum of $100/ month, an automatic payment is made from your credit card on file and added to your BonaVita Bank. Use funds from your BonaVita Bank for treatments like wrinkle relaxers, dermal fillers, facials and other in-office services or products. Plus, members earn discounts and receive special pricing.
BonaVita Members receive the following benefits:
Dysport, $7.00 per Unit (year-round discount)
Botox, $14.00 per Unit (year-round discount)
SkinBetter Science Skincare, 10% Off (year-round discount)
Colorescience Cosmetics, 10% Off (year-round discount)
BonaVita Beauty Skincare, 10% Off (year-round discount)
One free MICC-B Vitamin Shot (one per season)
One Full Syringe of Filler, $50 Off (one discount per season)
Facial Service, 10% Off (one discount per season)
Laser Hair Removal, 10% Off (one discount per season)

All your funds go directly towards treatments with no sign-up, membership or cancellation fee. The BonaVita team can make a payment recommendation based on your current injectable treatments to help you budget for your next treatment.
Your credit card will be automatically charged the 1st of each month and funds will be deposited to your BonaVita Bank. The membership fee must be paid via credit card. We will not accept debit cards or cash.
On the 1st of each month, our Electronic Medical Aesthetic Record system (Aesthetic Records) will automatically attempt to charge the credit card(s) on file. If the payment is declined, the system will automatically attempt to charge the card again on Day 2 and Day 3.
If payment fails on Day 1, our office will contact you via two forms of communication: phone, email, and or text to obtain updated payment information. A second attempt to reach you will be made on Day 2.
If you fail to respond to our attempts to update your payment method, your BonaVita Membership will be terminated. Any remaining balance in your membership wallet will not be refunded and will no longer be applicable toward retail purchases or in-office procedures.
Holiday or Office Closure Exceptions:
If the 1st of the month falls on a holiday or the office is closed for any reason:
The system will still attempt to charge your card automatically.
Our office will contact you for updated payment information on the next business day.
If we are unable to reach you on the next business day, we will make one final attempt on Day 3.
If updated payment information is not provided by Day 3, your membership will be terminated, and any remaining balance will not be refunded or applicable toward future retail purchases or in-office procedures.
If you do not have enough in your BonaVita Bank to cover the cost of your treatment, simply pay the difference at the time of your appointment. If your treatment costs less than the balance in your BonaVita Bank, the money will remain in your BonaVita Bank until next time. Seasonal discounts may not be rolled over and must be booked during the months of February, May, August and November of that calendar year. Money from your BonaVita Bank may be used at any time and will rollover. Funds are non-transferable.
BonaVita Membership Cancellation Policy
You may cancel your BonaVita Membership at any time. To cancel your membership, please email bonavitaaesthetics@gmail.com and provide one week notice. Our billing department will confirm cancellation with you. No other forms of cancellation will be considered. Funds in your BonaVita Bank must be used within 90 days of cancellation or BonaVita Aesthetics reserves the right to withhold money in your account.
BonaVita Membership requires a minimum commitment of 4 months. If you cancel before the minimum commitment is met, you will be charged for the difference.
Prices are subject to change without notice. No refunds will be provided.
In the chance of a life event, such as but not limited to, pregnancy or deployment, your membership perks and monthly payment will be paused until pregnancy is no longer indicated and/ or you are home from active duty. Each case will be reviewed and approved by owner/ manager.
